Evaluate the surface integral of a vector v=xi + yj+ zk over the closed surface of a cylinder w the circular base of radius a in the x-y plane and length h alon

As given in printed notes,how these equations came? (round marked)

    dear, unit vector in z direction and -z direction.... but we consider origin of coordinates on bottom so hight of cylinder in bottom surface is Zero and Upper is h hight.... so we only take upper part...

    but in second marked equation,we are not taking z.why?

    dear in second question not taking z bcoz..... u see diagram again... we take origin in end of cylinder.. no part exist of cylinder below origin ...see ,..so we can't take z here..
