The true velocity at any time t is dr/dt +w* r how this equation came from

    As we studied from our school days that velocity is dx/dt, that was the case of straight line motion. But in our real life we don't always walk on straight line from the reference of any point(origin). so for convince we use polar coordinate so in polar coordinate we get two component of velocity, the first one is radial and second transverse.
